Osteogenic Therapy For Relapsing Odontogenic Cyst Through Immediate Orthodontic Traction

This case explores treating relapsing large mandibular cysts with impacted teeth using marsupialization, partial cyst wall curettage, and early orthodontic traction. After 2-year follow-up observation,no recurrence occurs and new bone fills full in cyst. A 17 - year - old boy with a left mandibular relapsing cyst and impacted teeth 33, 34, and 35, who just had a surgery of marsupialization and curettage 2 years ago, but without impacted teeth traction. An orthodontic traction device was immediately bonded., 2-4 weeks soon after surgery, orthodontic traction was carried out. During 1-year treatment, the cyst healed, impacted teeth errupted, with properly teeth morbility and unsure root resorption. This approach prevents recurrence, maintains dental integrity, and promotes cyst healing through tooth movement. Intracapsular pressure changes during traction inhibit cyst fluid accumulation and wall growth, reducing recurrence risk. Teeth movement can stimulate peri-root bone regeneration, fill the cyst cavity, and facilitate cyst healing. Meanwhile, the impacted teeth develop normally, avoiding issues like excessive crown - root angulation, root curvature, and root - bone ankylosis. This significantly reduces the risk of tooth loss and underdeveloped jawbones. Marsupialization and curettage associated with orthodontac traction is expected to be a better therapy for jaw cys than mearly sugery.Orthodontic impacted teeth movement should be as earlier as possible after surgery with light and continuous force.
