Class II Mandibular Retrusion In Adolescent: Clear Aligner Treatment

Mandibular retrusion, a prevalent form of Class II malocclusion in adolescents, is often caused by occlusal interference or functional factors. Early elimination of such interferences during mixed dentition facilitates mandibular advancement, establishes stable overbite-overjet relationships, and improves facial aesthetics. An 8-year-old female presented with protruding teeth and a retrusive chin. No prior orthodontic treatment or oral habits were reported. Clinical evaluation revealed mild chin deviation, a convex profile, mandibular retrusion, and hypotonic lip musculature. Intraoral examination showed mixed dentition, bilateral full-distal molar relationships, severe deep overbite (Class III), severe overjet (Class III), narrow dental arches, and coincident dental/facial midlines. Treatment Design: 1. Maxillary expansion to eliminate occlusal constraints, combined with Class II guidance for anterior alignment. 2. Lower anterior intrusion and Spee curve leveling to enhance mandibular space, with torque control. 3. Vertical control (upper anterior/lower posterior intrusion) to counteract clockwise rotation. 4. Lip-cheek muscle training for stability. Post-treatment outcomes demonstrated stable overbite-overjet relationships and significant facial profile enhancement. Early clear aligner therapy in adolescents effectively addresses arch morphology, removes occlusal constraints, and harnesses mandibular growth potential. Integration with myofunctional training supports balanced three-dimensional craniofacial development. This case underscores the value of interceptive strategies during active growth phases to optimize skeletal and functional outcomes.

Abstract

Mandibular retrusion, a prevalent form of Class II malocclusion in adolescents, is often caused by occlusal interference or functional factors. Early elimination of such interferences during mixed dentition facilitates mandibular advancement, establishes stable overbite-overjet relationships, and improves facial aesthetics. An 8-year-old female presented with protruding teeth and a retrusive chin. No prior orthodontic treatment or oral habits were reported. Clinical evaluation revealed mild chin deviation, a convex profile, mandibular retrusion, and hypotonic lip musculature. Intraoral examination showed mixed dentition, bilateral full-distal molar relationships, severe deep overbite (Class III), severe overjet (Class III), narrow dental arches, and coincident dental/facial midlines. Treatment Design: 1. Maxillary expansion to eliminate occlusal constraints, combined with Class II guidance for anterior alignment. 2. Lower anterior intrusion and Spee curve leveling to enhance mandibular space, with torque control. 3. Vertical control (upper anterior/lower posterior intrusion) to counteract clockwise rotation. 4. Lip-cheek muscle training for stability. Post-treatment outcomes demonstrated stable overbite-overjet relationships and significant facial profile enhancement. Early clear aligner therapy in adolescents effectively addresses arch morphology, removes occlusal constraints, and harnesses mandibular growth potential. Integration with myofunctional training supports balanced three-dimensional craniofacial development. This case underscores the value of interceptive strategies during active growth phases to optimize skeletal and functional outcomes.
