Using An Autonomous Robot-Assisted Autogenous Tooth Transplantation

Autogenous tooth transplantation is a restorative technique that involves transplanting a patient's own tooth into an edentulous area, typically using a third molar to replace a nonfunctional molar, thereby "repurposing a discarded resource." In recent years, robotic surgical assistance has been widely applied in breast cancer surgery, liver cancer surgery, and orthopedic surgery, but its use in autogenous tooth transplantation within oral surgery remains relatively uncommon. This case highlights the application of an oral surgical robot in assisting autogenous tooth transplantation. A 29-year-old male patient underwent root canal therapy and post-core crown restoration for tooth 26. Imaging revealed low-density shadows at the apices of the palatal, mesiobuccal, and distobuccal roots, diagnosed as chronic apical periodontitis (post-root canal therapy). After failed intentional tooth replantation, the patient underwent extraction of tooth 26 and autogenous transplantation of tooth 28. Following preoperative planning of the socket preparation path, autonomous robot-assisted alveolar socket preparation for tooth 26 was performed. Concurrently, root canal therapy, apicoectomy, and retrograde filling were completed for tooth 28, which was then implanted and stabilized. At the 3-month postoperative follow-up, the transplanted tooth exhibited favorable gingival healing, physiological mobility, and reduced periodontal low-density shadows. Conclusion/clinical significance: his case demonstrates that artificial intelligence and robotic assistance can achieve precise, minimally invasive alveolar socket preparation in autogenous tooth transplantation. Combined with concurrent root canal therapy, apicoectomy, and retrograde filling, this approach may enhance the success rate of autogenous tooth transplantation.
