Combined Orthodontic-Periodontal Treatment For Iatrogenic Elongation Of Central Incisors
This case report describes a 12-year-old girl who developed abnormal elongation of the central incisors by 5mm during diastema closure using orthodontic elastic chains at a clinic. Subsequent periodontal basic therapy and combined orthodontic-periodontal intervention successfully restored dental morphology. A 12-year-old patient experienced 5 mm elongation of the two central incisors. Additionally, approximately one - quarter root exposure occurred. This was a result of standard orthodontic treatment using orthodontic elastics to close the diastema between the central incisors. The treatment plan included initial periodontal basic therapy, followed by orthodontic realignment. After two years of fixed orthodontic treatment, during which the incisors were aligned and intruded, the crown length was reduced, and a 2-mm gingival recession occurred. Subsequent gingival grafting surgery was performed to address the gingival recession. The patient’s dental and gingival conditions significantly improved after treatment. This case underscores the irreversible periodontal damage caused by non-standard orthodontic practices. Multidisciplinary collaboration ensured optimal therapeutic outcomes, providing a valuable reference for managing similar cases involving iatrogenic complications. This case highlights the importance of multidisciplinary collaboration between orthodontics and periodontics in managing iatrogenic complications, such as tooth elongation and gingival recession, to achieve optimal clinical outcomes and restore dental and periodontal health.
