Autotransplantation In Combination With Orthodontic Treatment

This case report describes an adult patient with multiple decayed teeth and openbite who underwent tooth transplantation surgery and orthodontic movement and were followed up for more than 3 years. A premolar was extracted and autotransplanted to replace another extracted hopeless premolar. A CBCT-derived 3D printed replica was used and achieved a positive result. Clinical examination (looseness, gingival morphology, depth of periodontal pocket, etc.) and CBCT examination were performed on transplanted tooth. The transplanted teeth showed no abnormalities, no significant movement, and the periodontal ligament space was normal. Autotransplantation can be a valuable treatment option which should be put in mind when the orthodontist plans treatment. The technology with a 3D print replica can be used for minimally invasive and predictable autologous tooth transplantation surgery, quickly preparing a matching alveolar cavity with the donor tooth, shorten the residence time of the transplanted tooth in vitro, and increase the success rate of autotransplantation. This article conducts interdisciplinary cooperation to carry out autotransplantation in a orthodontic patient with multiple decayed teeth, preserving her natural teeth and achieving positive results. The use of a CBCT-derived 3D printed replica is useful in reducing the surgical time and increasing success rate of transplantation.
