Multidisciplinary And Sequential Prosthetic Rehabilitation Of Bilateral Maxillary Defects

This report presents a multidisciplinary approach for the rehabilitation of a patient with bilateral maxillary defects, incorporating multi-dimensional functional evaluations and a 10-year follow-up to validate its effectiveness. A 43-year-old man was referred for a preoperative consultation, with computed tomography revealing tumor occupying the entire maxilla and extending to the skull base. The diseased tissue was resected, and five implants were placed in the zygomatic region in a single operation. The surgical obturator, interim obturator prosthesis, and definitive obturator prosthesis were provided immediately after surgery, at two weeks, and at three months postoperatively, respectively. Throughout the rehabilitation process, the patient carried out orofacial myofunctional exercises to adapt to the large prosthesis. A comprehensive set of multi-dimensional functional assessments was designed and implemented during prosthetic rehabilitation and follow-up visits over a 10-year period. The proposed multidisciplinary workflow is highly effective for rehabilitating patients with extensive maxillary defects, as it comprehensively addresses the complex needs of these cases. The primary implant placement protocol minimizes complications associated with subsequent radiotherapy, while orofacial myofunctional therapy helps prevent postoperative trismus. The sequential use of obturator prostheses eliminates the “cavity period”, facilitating early oral function recovery. Additionally, multi-dimensional functional assessments provide a structured framework for monitoring and optimizing functional rehabilitation. Repairing extensive maxillary defects requires a well-coordinated interdisciplinary approach. This report illustrates how experts from multiple disciplines can collaborate effectively to manage complex cases and address critical details throughout the rehabilitation process, ultimately ensuring successful restoration.
