Soft Tissue Augmentation For Aesthetic Zone Implant Complications

The success of anterior dental implants relies on high survival rates and long-lasting aesthetic outcomes. Common aesthetic challenges include gingival recession, papillae loss, and implant platform exposure, which demand effective and lasting solutions. The existing abutment and crown were removed, replaced with a smaller abutment for soft tissue contouring. A full-thickness flap extending from abutment to alveolar ridge was created, overlaid with a half-thickness flap. Root-side incisions and gingival resection (1.5 mm at position 11; 1 mm at 21) preceded connective tissue grafting. Grafts were 6 mm wider than the abutment, with SCTG positioned 2-3 mm subgingivally, aligned to adjacent margins and buccal bone crest. Graft thickness matched the flap (total >2 mm), debrided (0.3-0.6 mm), thicker centrally and tapered peripherally. SCTG effectively addresses peri-implant soft tissue deficiencies, offering durable aesthetic rehabilitation in anterior zones when conventional methods fail The successful application of SCTG in this case demonstrates its potential in addressing complications such as soft tissue collapse and discoloration around dental implants in the anterior aesthetic zone. This procedure offers a viable treatment option for patients experiencing aesthetic issues following long-term implant placement, particularly when traditional management strategies have failed.
