Incorporating Rural Health and Social Accountability in the Postgraduate Medical Curriculum: A Proposal for Structured Immersion and Evaluation Framework
Despite substantial reforms in undergraduate medical education in India, postgraduate (PG) medical training continues to be limited by approaches that are focused on urban areas and tertiary care, which do not adequately equip specialists for the challenges of rural healthcare.Although the National Medical Commission (NMC) requires a 3-month posting at a district hospital during PG training, the rural health system is still not sufficiently incorporated into the official curriculum. This article suggests the implementation of a structured Rural Health Immersion Program (RHIP) within the PG curriculum, which will be evaluated using a composite Rural Immersion and Social Accountability Index (RISAI).The index consists of five evaluative domains: community engagement, problem-solving, documentation, mentor/peer evaluation, and professionalism, all of which are objectively scored and analyzed. The goal is to cultivate a culture of empathy, system-level awareness, and context-sensitive clinical practice among the upcoming specialists in India.
