36: ECMO support for post Rickettsial infection and successful outcome
Introduction: Rickettsia is a group of vector-borne organisms that cause acute febrile illnesses throughout the world. While the clinical presentation of rickettsial infection is similar, the causative species and epidemiology can vary depending on the region. The infection of endothelial cells leads to disseminated inflammation, loss of barrier function, and altered vascular permeability throughout the body. Consequently, those infected develop fever, myalgias, central nervous system symptoms such as a headache and confusion, rash, and cardiovascular instability. Mortality can occur in severe cases of infection Case Presentations: An 8 year female patient was admitted to emergency ward in our hospital with rickettsial fever with severe rashes in the body with bulla and ecchymosis. She was shifted with oxygen support from a small clinic after 5 days. On evaluation she had septic shock with severe bi-ventricular dysfunction and liver dysfunction. The patient was hemodynamically unstable and decided to put on ECMO due to cardiogenic shock. the patient was initially put on Peripheral VA ECMO and then later converted to Central VA ECMO with LV venting. This rare case of this ECMO is discussed in this presentation along with the management Conclusion: Rickettsial fever patients with severe mortality can be recovered using Extra corporeal membrane Oxygenation support.
