49: Challenges during VV-ECMO in a severely obese patient with Influenza-A

Introduction: Severe obesity carries a high risk of morbidity & mortality for the patient during critical illness, and the care of such patients might be challenging in many aspects. This abstract highlights the challenges encountered in the care of a severely obese patient during vv-ECMO; such as cannulation; ECMO flow; medication dosing; and nursing care. Case Presentation: This patient is a 35-year-old severely obese woman with a height of 155 cm and weight of 185 kg (body mass index: 77kg/m2). admitted to the ICU as a case of severe CAP influenza A necessitated intubation and mechanical ventilation and eventually vv-ECMO because of refractory hypoxemia; the ICU stay was complicated by septic shock; and AKI required CRRT. The patient improved and de-cannulated from ECMO and discharged to physical and cardiopulmonary rehabilitation. Conclusions: This case report shares our experience with other centers to know the obstacles of caring for such types of patients and be ready with a clear organized plan to deal with them.
