Clinician-led Digital Innovations in Diabetes Care: Insights from the 2025 Jothydev’s Professional Education Forum–USV Technology Awards in India

Abstract Background: India faces a dual challenge in diabetes care: a rising prevalence and limited access to personalized management in semiurban and underserved regions. Community-originated, clinician-led innovations are increasingly addressing these gaps but remain underdocumented in academic literature. The Jothydev’s Professional Education Forum–USV Diabetes Technology Awards were established to recognize and support such field-level solutions, offering a repository of real-world approaches. Objective: To synthesize and describe the award-winning innovations recognized in the 2025 cycle, highlighting scalable, frugal, and culturally contextualized approaches to diabetes care. Materials and Methods: A descriptive thematic synthesis of more than 20 submissions was undertaken. Eligible entries included structured workflows, program documents, and multimedia evidence. Innovations were grouped into six thematic clusters: AI-enabled clinical decision support, digital therapeutics, hybrid telemedicine, social media empowerment, next-generation education interfaces, and translational biomarker research. Outcomes such as glycated hemoglobin (HbA1c), time-in-range (TIR), adherence, and patient engagement were descriptively summarized. Results: The innovations collectively demonstrated frugal, context-sensitive design and reported improvements in glycemic control, treatment adherence, patient empowerment, and community reach. Several programs documented HbA1c reductions, TIR gains, follow-up adherence exceeding 90%, and outreach campaigns engaging more than two million people. Conclusion: Community-originated, clinician-led innovations are reshaping diabetes care in India through practical, low-cost, and culturally adapted digital models. Recognition frameworks such as the Awards provide legitimacy and visibility, enabling pathways for validation, scale-up, and integration into broader health systems. These lessons extend beyond India, offering transferable insights for resource-constrained settings worldwide.
