Para-Psoas Hernia: Importance of Adequate Dissection in the Space of Bogros during Laparoscopic Groin Hernia Repair—A Case Report

Abstract Para-psoas hernia is a rare and often underdiagnosed entity, typically missed during open hernia repairs due to limited exposure. Diagnosis may require dynamic magnetic resonance imaging or may be identified intraoperatively during laparoscopic repair. We report the case of a middle-aged male with persistent right inguinodynia following an open right inguinal hernioplasty. The patient returned several months later with recurrent groin pain and swelling. Clinical examination confirmed a recurrent inguinal hernia. During laparoscopic trans abdominal preperitoneal repair, a direct inguinal hernia and a hidden para-psoas hernia were identified and managed with mesh repair. At one-year follow-up, the patient remained symptom-free with no recurrence. This case highlights the importance of thorough dissection in the space of Bogros to identify rare hernia types like para-psoas hernia and prevent chronic groin pain.
