Optimizing Nephrology Care: Collaborative Roles of Advanced Practice Providers at Penn State

Background: The complexity of nephrology care demands innovative, team-based approaches to meet rising patient needs and optimize outcomes. Advanced Practice Providers (APPs) are essential members of the nephrology care team, enhancing efficiency, promoting continuity of care, and creating a collaborative learning environment. At our Division of Nephrology, APPs contribute significantly across inpatient, outpatient, and transitional care settings. Methods: We conducted a descriptive analysis of the roles and contributions of APPs within the nephrology division, based on workflow mapping, and review of care processes across inpatient, outpatient, and post-discharge settings. Key domains evaluated, inpatient dialysis coordination, acute follow-up, outpatient CKD care, and support for nephrology fellows. Results: Inpatient Dialysis Management: managed pre-rounding, lab review, dialysis coordination, and triaged urgent pages. This allowed fellows to focus on complex cases and academic growth. They also ensured timely interventions for complications such as access dysfunction or intradialytic instability through collaboration with nursing, vascular, and dialysis teams. Post-Discharge Follow-Up: facilitated smooth hospital-to-home transitions by arranging dialysis, addressing barriers, and educating patients on vascular access, diet, and fluid management. They intervened in cases of access issues or symptom relapses, helping to reduce emergency visits and readmissions. Fellow and Team Support: handling routine clinical duties, empowering fellows to concentrate on research and advanced patient care. They participated in case discussions and improved overall team communication. Outpatient CKD Management: independently managed stable CKD patients, adjusting medications, reviewing labs, and providing personalized counseling. Their involvement improved clinic access, reduced wait times, and maintained care quality. Conclusion: At Penn State’s Division of Nephrology, APPs are pivotal in delivering efficient, patient-centered care. Their roles in inpatient dialysis, post-discharge care, CKD management, and fellow support demonstrate the transformative impact of APPs on workflow, patient outcomes, and education. These findings support broader integration of APPs in nephrology practice nationwide.

Advanced Practice Providers (APPs) are essential members of the nephrology care team, enhancing efficiency, promoting continuity of care, and creating a collaborative learning environment.
