Effects of bisphosphonates & denosumab on bone mineral density in patients with osteoporosis - A study from central India. Introduction - Osteoporosis & fragility fractures are extremely prevalent, associated with significant Mortality &, morbidity and health care costs. It is more prevalent among women than men. More than 90% of individuals in our country who sustain fracture are never evaluated, investigated and treated. We analyzed 63 individuals with osteoporosis with various treatment modalities. Aims & Objectives - To assess the effects of alendronate, zolendronate & denosumab in osteoporosis individuals. Methodology- This is a retrospective study included 63 osteoporotic individuals aged 44-77 years. Patients attending Apollo sugar clinic with osteoporosis were included. Three groups of treatment alendronate- N=13, Zoledronic acid- N=39 & denosumab- N=11 allocated. BMD was assessed by dual energy x-ray absorpsiometry, at baseline and one year completion of therapy. T -score: Normal: -1.0 or higher, osteopenia- -1.0 to -2.5 & osteoporosis -2.5 or lower. Statistics: - Non-parametric test (Wilcoxon-rank test) and descriptive test( mean, median and percentage change) used for data analysis. Results: - 63 patients met the inclusion criteria. Mean age was 56-62. 49 patients had diabetes mellitus (72%), however its correlation with the severity could not be assessed. Significant improvement (p <0.001) found in T-score among IV zoledronate group Pre-treatment lumber spine (Mdn = -2.4, n = 39) Vs Post treatment lumber spine (Mdn = -2.2, n = 39) respectively, pre- treatment and post treatment T score were found in pre-treatment right hip (Mdn = -2.1, n = 39) & Post treatment right hip (Mdn = -1.9, n = 39)] and Pre-treatment left hip hip (Mdn = -2.1 , n = 39) & [Post treatment left -hip (Mdn = -1.8 , n = 39)] respectively. Significant improvement (p=0.002) found in Alendronate group, in lumber spine [ Pretreatment (Mdn = -2.2, n =13) & Post-treatment (Mdn = -1.9, n =130)], p-value =0.00148 in right hip Pre-treatment (Mdn = -1.7, n = 13) and Post treatment (Mdn = -1.4, n = 13), p-value = 0.002 left hip Pre-treatment (Mdn = -1.7, n = 13) and Post treatment (Mdn = -1.4, n = 13) respectively. Denosumab resulted significant improvement (p < 0.05), Pre-treatment (Mdn = -2.5, n = 10) and Post-treatment (Mdn = -2.3, n = 10) respectively. Right hip had significant (p=0.007) improvement in Pre-treatment (Mdn = -2, n = 11) and Post-treatment (Mdn = -1.8, n = 11) respectively. No improvement (p=0.109) in left hip Pre-treatment (Mdn = -2, n = 11)] and [Post-treatment (Mdn = -1.9, n = 10)] group. Conclusion: - All three drugs resulted significant improvement except in Denosumab group in the left hip. For better understanding more sample size & long-term outcome studies are required. Treating such patients had resulted rewarding results. Presentation: Saturday, July 12, 2025
