MON-580 Two-hour glucose criterion is first to detect diabetes in non-obese people from Korea

Abstract Disclosure: D. Kim: None. M. Im: None. S. Ryang: None. J. Kim: None. J. Ha: None. S. Kim: None. Background: The current ADA diagnostic criteria for type 2 diabetes (T2D) are fasting glucose ≥126 mg/dl, or two-hour glucose≥200 mg/dl, or HbA1c≥6.5%. Diagnostic discrepancy between criteria is one of the challenges for early detection of patients who are at high risk for diabetes. However, clinical studies of diagnostic discrepancy have been based mostly on cross-sectional data. Objectives: (1) identify which of the three criteria first detects diabetes, (2) estimate equivalent values of other criteria when one is diagnosed by the earliest criterion, and (3) estimate time lags of other criteria to reach their thresholds. Methods: Here, we investigated a 14-year longitudinal data set (N=230) in a non-obese Korean cohort where all participants (age, 50.5±7.7 years; BMI, 25.0±3.0 kg/m2 at baseline) progressed from NGT to PreDM to T2D. Linear Mixed Effect Models were used to estimate equivalent values of criteria and time lags between criteria. Results: 80% of the participants (183/230) developed diabetes by first passing the two-hour glucose criterion, 200 mg/dl. The fasting glucose and HbA1c values were 107 mg/dl and 5.9 %, respectively when the two-hour glucose was 200 mg/dl. Fasting glucose and HbA1c passed their respective thresholds 15 years later. Conclusion: Two-hour glucose should be routinely checked with those who are at high risk for diabetes in non-obese people from Korean. Presentation: Monday, July 14, 2025
