Objective: To investigate the factors affecting the prostate-specific antigen (PSA) components (total serum PSA levels, free PSA (fPSA) levels, and fPSA/PSA ratio) in men with diabetes and comparing these components between patients with and without diabetes. Methods: In this case-cohort study, a total of 1586 men (984 patients with type 2 diabetes (T2D) and 602 individuals without diabetes) who met the inclusion criteria were evaluated for PSA components from 2015 to 2020. Results: In this retrospective cohort study, PSA levels were lower in the T2D group than in the control group. According to the Appropriate Definition of Metabolic Syndrome among the Iranian Adults Report criteria, patients with metabolic syndrome had significantly lower fPSA levels than those who did not have metabolic syndrome (0.44±0.33 ng/mL vs. 0.51±0.37, p-value =0.009). Moreover, there was a linear relationship between age, lipid profile, body mass index (BMI), and PSA and fPSA levels (p-value= <0.001). Furthermore, there was a linear relationship between age, low-density lipoprotein cholesterol, and diabetic nephropathy with fPSA/PSA ratio. Conclusion: The present study showed that fPSA/PSA ratio in patients with T2D is less influenced by diabetes, blood pressure, metabolic syndrome, non- alcoholic fatty liver disease, liver enzymes, duration of diabetes, and BMI when compared to PSA and fPSA levels. Thus, using the fPSA/PSA ratio in patients with T2D, especially among individuals older than 60 years, could improve the accuracy of screening in prostate cancer detection.
