Currently, non-alcoholic fatty liver disease (NAFLD) is becoming an increasingly common disease among young people with a deficiency of fat mass, which is a consequence of estrogen deficiency, which develops against the background of reduced nutrition, which leads to iron deficiency and impaired steroidogenesis. Objective. Find the relationship between iron deficiency and estrogen, which leads to NAFLD. Materials and methods. The study included 20 patients with, aged from 18 to 45 years, average age – 35.87±10.76 years, gender distribution: men 5 (75%), women 15 (75%). Criteria for inclusion in the study: patients diagnosed with a deficiency of fat mass which determines according to bioimpedance analysis of body weight composition (BABWC); with iron deficiency, established by biochemical blood analysis: total iron binding capacity of serum (TIBCS), ferritin, serum iron. The level of estrogens is determined by steroid profile of saliva.Results. As a result, in all 20 patients, BABWC determines weight and fat mass deficiency, according to biochemical blood analysis: ferritin 25.45±8.34 ng/ml (iron deficiency anemia is ferritin < 45 ng/ml (by American Gastroenterological Association), serum iron 5.61±3.54 mmol/l, TIBCS 69.56±14.67 mmol/l. The estrogen level is 3.25±0.78 pg/ml (reference from 6.0 pg/ml). According to the ultrasound data: fatty liver infiltration.Discussion. Patients with reduced nutrition develop not only a deficiency of body weight and fat mass, but also develop iron deficiency, and in these 20 patients - iron deficiency anemia, which in turn leads to a decrease in the work of enzymes involved in the synthesis of steroid hormones, and the most pronounced decrease in estrogen levels. Estrogens are the main hepatoprotectors. In women, estrogen deficiency is manifested by dry skin, mucous membranes (vagina), dyspareunia (pain during sexual intercourse), hair loss and thinning, and poor menstruation. Men also have dry skin, hair loss and thinning, and increased blood pressure. But estrogen deficiency also leads to increased lipogenesis and decreased lipolysis in liver cells, contributes to the development of oxidative stress, insulin resistance, inflammation, increased and activated fibrosis, and the development of autoimmune lesions.Conclusion. Thus, a new diagnostic criterion and a new diagnosis appear: in patients with NAFLD nutritional deficiency which leads to body and fat weight deficiency. Presentation: Saturday, July 12, 2025
