Despite advancements in glucose monitoring and insulin therapy, healthcare disparities affect management, contributing to its significant economic and public health burden. Method: This study is the first to estimate the incidence, prevalence, disability-adjusted life years (DALYs) and years lived with disability (YLDs) due to T1DM across the EU from 1990 to 2021, stratified by age, sex, year, and location, using the Global Burden of Disease Study 2021. Results: From 1990-2021, the total prevalence due to T1DM increased from 1.3 (95%Uncertainty Interval: 1.5-2.2) million to 2.9 (2.5-3.4) million, DALYs increased from 237,984 (208,453- 275,611) to 283,754 (212,677- 371,324), while deaths decreased from 4,454 (4,315- 4,569) to 2,454 (2,290- 2,575). From 1990 to 2021, Cyprus experienced the highest annual percentage change (APC) in age-standardized prevalence rate (ASPR) at 2.88%, followed by Ireland (2.74%), Greece (2.27%), France (2.2%), Italy (2.1%), Croatia (2.06%), and Portugal (1.87%). The highest increase in age-standardized incidence rate (ASIR) was also observed in Cyprus (2.84%), followed by Ireland (2.65%) and Greece (2.37%). In contrast, age-standardized mortality rate (ASMR) declined in most countries, with Slovenia showing the highest decrease (4.17%), while Lithuania was the only country to record an increase (0.015%). The highest APC in age-standardized DALYs rate (ASDALR) was observed in Greece (1.37%), followed by Italy (1.16%), Ireland (1.06%), Spain (0.7%), Lithuania (0.67%), and France (0.51%), whereas Slovenia and Poland had the greatest declines. By age group, the highest number of new T1DM cases in 2021 occurred in children aged 5-9 years (6,170 cases, 95% UI: 4,327-8,489), followed by the 10-14 age group (6,084 cases, 95% UI: 3,710-8,628). Mortality was highest among those aged 65-69 years (277 deaths, 95% UI: 258-296), followed by the 75-79 age group (271 deaths, 95% UI: 241-295). The highest DALY burden was observed in the 55-59 age group (27,729 DALYs, 95% UI: 20,461-37,410), followed by those aged 40-44 years (26,512 DALYs, 95% UI: 21,115-33,686). Gender-wise, the overall burden was higher in males than females. The total percentage change in incidence was greater in males (83%) than females (68%), as was the increase in DALYs (26% vs. 12%). However, the decline in deaths was more pronounced in females (61%) compared to males (28%). Conclusion: From 1990 to 2021, the prevalence and DALY burden of T1DM in the EU nearly doubled, while mortality rates declined in most countries. The rising incidence, particularly among younger age groups and males, underscores the growing public health challenge, emphasizing the urgent need for targeted prevention, improved disease management, and equitable healthcare access across the region.
