MON-182 An Uncommon Complication Of Polycystic Ovarian Syndrome

Abstract Disclosure: M. Balogun: None. J. Kaur: None. Background: Polycystic ovary syndrome (PCOS) is a complex condition. It is characterized by elevated androgen levels, menstrual irregularities, and/or small cysts on one or both ovaries. It is associated with some clinical manifestations including menstrual irregularities and infertility.Although menstrual irregularities and infertility are a common association of PCOS, the complications extend beyond menstrual irregularities and infertility. PCOS has been associated with an increased risk of developing type 2 diabetes mellitus, dyslipidemia, hypertension, cardiovascular disease, endometrial cancer, and depression. This case report is about an uncommon complication of polycystic ovarian syndrome.CaseThis is a case of a 34-year-old female who presented with left lower leg pain and swelling that started about 6 days prior. The patient stated that her symptoms began suddenly with left lower leg pain. Her symptoms gradually worsen with increased leg swelling and erythema. She denied being pregnant. The patient stated that she was diagnosed with PCOS 6 years ago. The last time she took OCP was 5 years before presenting to the hospital. She had no other medical conditions. A duplex ultrasound of her left lower extremity revealed an occlusive deep vein thrombosis in the left popliteal vein. A CTA of the chest was concernng for pulmonary embolism. Subsequently, a CTA of the pelvis was performed, which was also concerning for multiple fibroids. Due to the heavy vaginal bleeding and also venous embolism, interventional radiologists performed uterine artery embolization and placed an IVC filter. 24 hours after the procedure and with stable hemoglobin, the patient was started on an anticoagulant.DiscussionVenous thromboembolism (VTE) is characterized by the presence of a blood clot (thrombus) in the deep vein, and /or pulmonary embolism. Unlike menstrual irregularities, infertility which are common complications of PCOS, VTE is usually not reported as a complication of PCOS. This patient with no previous history of venous embolism, or other known risk factor for venous thromboembolism makes PCOS indicative of the etiology of this patient’s venous thromboembolism. This case study underscores the importance of considering venous thromboembolism as a differential diagnosis in women with PCOS experiencing leg swelling, and difficulty breathing, particularly when other common causes have been excluded.
