Multifunctional Serial Static Orthosis for Metacarpophalangeal Joint Contractures
ABSTRACT Background Metacarpophalangeal joint flexion (MCPJ) stiffness leads to functional impairment and discomfort. Serial static orthoses can address effectively joint contractures by providing controlled force over time, promoting tissue elongation and joint mobility. Objective Addressing the MCPJ stiffness through a practical nondynamic orthosis, which promotes tissue elongating and improves joint mobility, while allowing function and minimizing discomfort. Study Design Descriptive technical note analyzing the design and application methods of the proposed orthosis, which arise from the clinical experience while treating patients with MCPJ stiffness after trauma. Technique In the described fabrication process, the proposed orthosis is easy to fabricate and lightweight. Contains adjustable components, which provide controlled static force to the MCPJ, facilitating targeted stretching of contracted tissues. The pattern can be used as a static progressive orthosis to address PIP and DIPJ joint flexion stiffness, as well as a static exercise orthosis for PIPJ extension lag. Key elements are joint end-feel and the right doses of low-load stress adequate to elongate the shortened tissues. Results The proposed design can improve MCPJ flexion stiffness and has multifunctional use for interphalangeal joint stiffness and PIPJ extension lag, capable to facilitate stiffness while allowing for function. Conclusions The orthosis can an effective tool improving passive MCPJ flexion in an effective manner. Its multifunctional use can have further clinical applications, assisting mobility for other joints too. Further research and clinical evaluation are warranted to validate the efficacy and long-term outcomes. Clinical Relevance The orthosis is a simple design, multifunctional, and patient-friendly solution, which can address MCPJ contractures, digital joint stiffness, or PIPJ extension lag. The adaptable and low-load pain-free application can improve clinical outcomes and compliance making it a considerable solution to long-term stiffness in hand therapy protocols.
