#1828 Albuminuria and coronary artery calcium in a general Japanese population

Abstract Background and Aims Both albuminuria and coronary artery calcium (CAC) are well-established cardiovascular risk factors. However, the relationship between albuminuria and CAC remains controversial. This study aimed to investigate their association in a general Japanese population. Method This was a population-based, cross-sectional observational study utilizing annual health check data collected between 2019 and 2022 in Nakagawa City and Fukuoka City, Japan. A total of 599 individuals aged 30 years or older who underwent both urine testing and CAC scanning were included in the analysis. Albuminuria was assessed using the urine albumin-to-creatinine ratio (UACR). Participants were categorized into three groups based on UACR tertiles: tertile 1 (<28.6 mg/g), tertile 2 (28.6–58.3 mg/g), and tertile 3 (>58.3 mg/g). CAC was measured using multi-detector computed tomography, and CAC scores were calculated according to the Agatston method. The presence of CAC was defined as a CAC score >0. Logistic regression analysis was conducted to estimate odds ratios (ORs) and 95% confidence intervals (CIs) for CAC in relation to albuminuria. Results Among 599 participants (mean age: 65.4 ± 8.3 years), 295 (49.3%) were women and 327 (54.6%) had detectable CAC. The prevalence of CAC increased across albuminuria tertiles (p for trend = 0.014). After adjusting for traditional cardiovascular risk factors, the ORs for CAC were 1.09 (95% CI, 0.69‒1.70) for tertile 2 and 1.77 (95% CI, 1.10‒2.84) for tertile 3 compared with tertile 1. A significant dose-response relationship was observed between albuminuria and CAC (p for trend = 0.019). Conclusion Albuminuria was significantly associated with an increased risk of CAC in a general Japanese population.
