Redo frozen elephant trunk procedures after surgical treatment of acute aortic dissection type I
Objective. To evaluate the results of redo frozen elephant trunk (FET) procedures after previous surgical correction of acute aortic dissection type I. Material and methods. A retrospective cohort analysis included 97 patients with DeBakey type I aortic dissection who underwent FET procedures between 2016 and 2025. Patients were divided into two groups depending on intervention: group 1 (n=45) — repeated patients who underwent previous surgical correction of acute type I aortic dissection, group 2 (n=52) — patients with acute type 1 aortic dissection who underwent previous radical treatment. Early postoperative results were analyzed. Results. Significant differences were obtained for some parameters: length of hospital-stay (p=0.0471), intraoperative parameters, incidence of infectious complications (p=0.028), recurrent laryngeal nerve injuries (p=0.001), as well as total number of complications (p=0.019). In addition, there was significantly higher incidence of distal aortic repair in the group of repeated interventions (p=0.033). Conclusion. Redo aortic arch surgery is still difficult with high risk of complications and negative distal aortic remodeling despite acceptable results in experienced centers.
