Successful Therapy of 2 Esophagobronchial-Tracheal Fistulas After Esophagectomy by Endoscopic Vacuum Therapy and Bronchial Stenting

Development of an esophagobronchial-tracheal fistula is a rare but severe complication of esophagectomy. There is no standardized treatment. We present the cases of 2 patients with esophagobronchial-tracheal fistulas after anastomotic leakage after esophagectomy with gastric tube reconstruction. We successfully treated these fistulas by implanting covered airway stents and by performing simultaneous esophageal endoscopic vacuum therapy (EVT). As a precondition for EVT, we provided a solution for compartmentalization between the wound cavity and the tracheobronchial system by performing bronchial-tracheal stenting and esophageal EVT in both cases. We propose a minimally invasive therapeutic alternative in the therapy of esophagobronchial-tracheal fistula after esophagectomy.
