Phenotype of arterial hypertension and vibration disease in patients with post-COVID syndrome

The aim of the study: based on the study of clinical-functional and sanitary-hygienic indicators, to identify the features of the phenotype of arterial hypertension (AH) and vibration disease (VD) in post-COVID syndrome. The study included 289 patients with stage I–II AH of moderate and high risk. The main (I) group consisted of individuals with AH and VB from exposure to local vibration who had previously worked at an aircraft enterprise and had been infected with COVID-19 (107 people); II — with AH and VB, not affected by NCI (86 people) and III — patients with AH, who had been infected with NCI (96 people). The second and third groups were comparison groups. The assessment of systemic hemodynamics was carried out using the methods of 24‑hour blood pressure monitoring and echocardiography. Results. In all patients who had COVID-19, a mild form of the infection prevailed (75.7 % and 74.0 %). In the main group of patients, an uncontrolled course of the disease was significantly more common (p<0.001) (52.3 %); a high frequency of refractory hypertension (10.3 %) and failure to achieve the target blood pressure level (61.7 %). A study of the daily blood pressure profile (DBP) in patients with AH and VB who had COVID-19 12 months after infection showed that all parameters of the DBP were exceeded during the day and at night. However, the higher values were observed for diastolic blood pressure. The main group had a higher frequency of nocturnal hypertension (Night-pickers variant — 9.3 %). Three months after the NCI, echocardiographic changes were noted in patients with AH and VB (an increase in the size and volume of the left heart, an increase in the mean pressure in the pulmonary artery, and a slight decrease in the stroke volume and ejection fraction of the left ventricle). Twelve months after the COVID-19 infection, patients in the main group continued to experience an increase in the size and volume of the left heart.
