Assessing the impact of mandibular third molars on mandibular incisor crowding: A comprehensive review
This review evaluates the evidence on the relationship between mandibular third molars and crowding of the mandibular incisors. While it has long been believed that impacted third molars cause late anterior crowding and narrowing of the dental arches, recent studies present conflicting results. A synthesis of observational studies, systematic reviews, and meta-analyses reveals that most do not support a strong causal link between third molar presence or impaction and mandibular incisor crowding. These findings suggest that current evidence does not substantiate the prophylactic removal of third molars to prevent crowding. Clinical management should be individualized, considering each patient’s conditions and oral health. Further high-quality, long-term research is needed to clarify the role of third molars in post-orthodontic relapse and support evidence-based clinical decision-making.
