Oral manifestation of metabolic condition. Clinical case

Manifestations of metabolic-associated conditions in most cases is reflected on the mucosa of the oral cavity, which confirms the systemic role of sedative chronic inflammation. This article presents a case from practice where dental manifestations allowed for insulin resistance and dyslipidemia for the first time. Case presentation. Patient with complaints of discomfort in the language and unusual its view. After examination and additional dental examinations, the diagnosis was black hairy tongue, oral lichen planus, reticular type. Hyposalivation (significant hypofunction of the salivary glands). This clinical picture caused caution, and the patient was referred to a physician with subsequent diagnosis: first-degree obesity (BMI = 30.59), insulin resistance syndrome, dyslipidemia, arterial hypertension degree 1, risk 2 (moderate). General treatment (lifestyle correction, dietary intervention, physical activity optimization, β1-adrenergic receptor blocker HMG-CoA reductase inhibitor, hypoglycemic agents) and local treatment (after professional oral hygiene with home hygiene, partial removal of keratinized cells and hyperplastic changed filiform papillae using enzyme solution application. Recommendations: moisturizing oral gel). Subsequent observation of the patient showed a positive trend in oral manifestations. Conclusion. Pathological manifestations on the mucosa of the oral cavity are an important clue for diagnosis, treatment and prevention of systemic disease.
