Post Extubation Negative Pressure Pulmonary Edema After An Appendectomy

Negative pressure pulmonary edema (NPPE) is a rare complication after an appendectomy surgery with non-cardiogenic origin. NPPE incidence ranged from 0.1% to 12% and mortality rate ranged from 2% to 40%. Young, athletic, healthy, and had smoking habit patients who undergoes endotracheal intubation are at high risk developing NPPE. Our case present male, 21 y.o., with NPPE post an appendectomy procedure. This patient has no previous medical history but can be classified as obese. Total length of stay was 8 days. Adequate management were delivered and patient was able to discharge with significant improvement. NPPE diagnosis requires the combination of clinical manifestations, supporting examinations, as well as appropriate clinical judgement. Management depends on severity, etiology, and mostly supportive measures. It usually improved within 12 – 48 hours. In general, patient in this case reflect a tipical characteristics, management, and prognosis in accordance with literatures.
