Een pulmonale caviteit valt niet te negeren
A pulmonary cavity cannot be ignored A cavity in the lung can be caused by subacute invasive aspergillosis. If not recognized in time, it can spread to the surrounding tissue, leading to fungal arthritis of the sternoclavicular joints and osteomyelitis of the clavicle. This article describes the case of a 61-year-old woman who presented with swelling and redness of the sternoclavicular joint bilaterally. The culture from the puncture was positive for Aspergillus fumigatus. She had a known history of atelectasis of the left upper lobe, which occurred after chemotherapy and radiotherapy for a carcinoid tumor 9 years prior. The last CT scan, 6 months ago, showed the development of a cavity in the left upper lobe. Now there was a progressive thickening of the cavity wall with the development of fistulisation to 2 subcutaneous collections, an underlying arthritis of the sternoclavicular joints and osteomyelitis of the left medial clavicle. Treatment with voriconazole was instituted with favourable evolution as a result.
