An increase in cardiac troponin T levels after transvenous lead extraction

Initially, an elevated level of c-TnT >14 ng/l was detected in 57.32% patients, then at 2 hours in 96.99%, and at 8 hours in 98.19% (164 [98.80%] patients). After TLE c-TnT levels increased significantly from (median 17.0; interquartile range 9.00-30.00) ng/l to 66.50 (36.00-153.0) ng/l at 2 hours and to 81.5 (45.00-179.0) ng/l at 8 hours after the procedure (P <0.001). The increase was due to a greater procedure complexity, and contrary to the initial level it had no impact on one-year survival (87.95% vs. 89.16%; P = 0.81). The one-year mortality rate in patients with c-TnT(0) levels >14 ng/l was higher than in those with c-TnT(0) ≤14 ng/l (16 [17.02%] vs. 3 [4.17%]; P = 0.01, respectively) CONCLUSIONS: Elevated levels of c-TnT(0) is an unfavorable prognostic indicator. Levels of c-TnT at 2 and 8 hours after the TLE procedure were elevated in most patients. Delta c-TnT(2) and c-TnT(8) were related only to the complexity of the procedure. TLE had some effect on the heart, causing troponin release, which may complicate the diagnosis of coronary syndromes in the first days after transvenous lead extraction.
