Introduction: posterior reversible encephalopathy syndrome (PRES) is a rare radiological and clinical entity characterized by neurological signs with reversible bilateral cerebral radiological abnormalities. The objective was to describe the role of magnetic resonance imaging (MRI) in its diagnosis. Case 1: a 26-year-old woman was admitted to the Clinique Les Etoiles in Bamako on March 24, 2023, for a cerebral MRI to assess headaches and hallucinations on day 5 postpartum, with a blood pressure of 170/100 mmHg. The MRI examination revealed multiple, nearly symmetrical bilateral fronto-parieto-temporo-occipital cortico-subcortical areas, lenticular, and right cerebellar areas with T1-weighted hypointense, T2-weighted hyperintense, and FLAIR images, with post-treatment regression. Case 2: This was a 47-year-old man with no known medical history. He was seen in the Radiology Department of the Luxembourg University Hospital on February 12, 2024, for a brain MRI to assess cortical blindness that had been progressing for approximately two months. The MRI revealed bilateral, symmetrical occipital and right temporal cortico-subcortical areas with T1-weighted hypointense, T2-weighted hyperintense, and FLAIR images, with a high diffusion coefficient (ADC), which resolved after medical treatment. Conclusion: posterior reversible encephalopathy is a little-known condition. MRI is a crucial tool in its diagnosis.
