Indications et profil des anomalies à l’échographie transfontanellaire (ETF) au service de radiologie du centre hospitalier national d’enfants Albert Royer de Dakar

Introduction: cerebral or transfontanellar ultrasonography is a non-irradiating, readily available and accessible imaging technique used as a first-line method for morphological and vascular assessment of the neonatal brain, particularly in premature infants. It is the technique of choice for postnatal brain exploration, even if MRI is the gold standard. The general aim of this study is to review the activity of ETF and to contribute our experience in the management of encephalic anomalies in children at the Albert Royer National Children's Hospital. Materials and patients: This was a cross-sectional descriptive study over a seven-year period from January 1, 2016 to December 31, 2022. We included all patients under 15 years of age who received ETF during the study period. We collected 682 patient records, including 375 girls and 307 boys, with a sex ratio of 0.82. The mean age was 12 days, with extremes of 1 and 720 days. The parameters studied were indications, source of prescriptions, elementary semiology and diagnosis. Data were entered and analyzed using Excel 2019 and Epi-info version 7.2.5. Results: Prescriptions came from CHEAR pediatricians (63.45%). Indications for perinatal asphyxia accounted for 22% of cases, followed by poly malformative syndrome (12%) and convulsions (10.26%). Results were pathological in 37% of cases. Hydrocephalus accounted for 18%, including 47% tetra ventricular hydrocephalus. Anoxic-ischemic encephalopathy was observed in 17% of cases. Cerebral atrophy was noted in 10.1% of cases. Malformations accounted for 10.4% of cases. Traumatic lesions were observed in 11.4% of cases. Infections accounted for 3.5% of cases. The other pathologies were cortico-subcortical atrophy 8.46% (26 patients), serosanguineous nodules 3.9% (12 patients), sinovenous thrombosis 0.65% (2 patients), parenchymal calcifications 1.30% (4 patients), and one case of choroid plexus cyst. Conclusion: The ETF remains the examination of choice in the initial assessment of brain pathology in newborns in our regions.
