Use of a virtual care platform to reduce oncology provider time toxicity: Operational and workforce modeling outcomes.

604 Background: Time toxicity—initially describing patient burden—now critically impacts oncology providers. Increasingly complex treatments have led to overwhelming volumes of messages, visits, and documentation. Burnout, administrative overload, and poor work–life balance are accelerating workforce attrition. Up to 40% of nurses plan to leave direct care within five years, and losing one oncologist can cost over $1 million in turnover-related expenses. Scalable, sustainable solutions are urgently needed. Methods: We assessed the operational impact of Reimagine Care’s virtual care platform, which integrates a text-based, rules-based virtual assistant (“Remi”) with a Virtual Care Center (VCC) staffed by oncology-trained MAs, RNs, and APPs. Remi uses natural language processing and modified ONS symptom pathways to triage patient concerns, escalating to human clinical staff based on internal thresholds and acuity modeling. We measured time savings per patient per month across a panel of 1,000 oncology patients (500 on active treatment), then translated this into labor offsets using U.S. salary benchmarks and escalation rate assumptions derived from literature and internal workflow data. Results: The platform saved 1.25 hrs/pt/month inclusive of Remi+VCC totaling 15 hours/year per patient—or 7,500 hours/year for an oncologist managing a 500-patient active treatment panel. Assuming 75% RN productivity (1,440 clinical hrs/year), this equals 4.9 RN FTEs. Applying escalation rates (20% RN→APP, 5% APP→MD, 5% RN→MD), we calculated additional savings of 1,400 APP hrs/yr. For an oncologist, this translates to 280 hrs/yr or ~50 minutes reclaimed per oncologist per workday. Conclusions: Reimagine Care’s tech-enabled, human-led model meaningfully reduces provider time toxicity by streamlining symptom triage and communication. While results reflect high-throughput panels, 40–60% of patients in typical practices may benefit. This approach helps preserve oncology workforce capacity and care quality at scale. Annual time and cost savings from reimagine care virtual platform per oncologist panel (n = 1,000 patients, 50% on treatment). Provider Role Annual Hours Saved Labor offset (FTE's) Estimated annual labor value 1 Registered Nurse 7000 4.90 $588,000 APP (NP/PA) 1400 0.97 $155,000 Oncologist 280 0.17 $75,000 Total — — $813,000 1 Salary + benefits: RN = $120,000/year; APP = $160,000/year; Oncologist = $550,000/year.
