Empowering equity: Implementing an SDOH dashboard across City of Hope's national oncology network background.

622 Background: City of Hope, a nationwide oncology health system, serves patients in many different geographic locations. City of Hope’s mission is to increase access to care and improve outcomes for people with cancer or diabetes. Social determinants of health (SDOH) can significantly impact patient outcomes. Our organization noted a lack of comprehensive data on SDOH, hindering targeted interventions. This gap is evident in both inpatient and outpatient settings, where understanding the influence of factors like housing instability, food insecurity, and transportation barriers is crucial for personalized care. Additionally, disparities in SDOH outcomes are often observed across different racial and ethnic groups, underscoring the need for stratified data to address health inequities effectively. Methods: To address this, the organization leveraged the electronic medical record to collect the data using a systematic way where the information could be accessed for reporting. Next, a system-wide SDOH dashboard was developed, integrating data from the health record as well as patient demographics information. The dashboard was designed to: ensure Process: Measure the capture of SDOH domains by site to make sure the process was working; visualize SDOH Data: Present data on social needs across various domains, such as food insecurity, transportation barriers, and housing instability; and track Referrals: Monitor the percentage of patients referred to social work based on identified needs. Data governance practices were implemented to ensure accuracy, privacy, and compliance with healthcare regulations. Results: The dashboard revealed: High Capture Rates for Inpatient: A significant increase in the percentage of patients that were assessed for SDOH; Data-Driven Decisions: Dashboard can be utilized to tailor interventions, improving patient satisfaction and outcomes; and Opportunities: Different opportunities can be identified and stratified by site/region, race, ethnicity and primary language. Conclusions: Implementing an SDOH dashboard enabled the health system to bridge data gaps, leading to more informed decision-making and enhanced patient care. By integrating stratified data, the system fostered a holistic approach to addressing social determinants, ultimately contributing to better health equity and outcomes.
