The Effect of Dressings with Granulated Sorbent and Modern Antiseptic Agents on the Treatment of Wound Defects in Patients with Diabetic Foot Syndrome

INTRODUCTION: Treatment of wounds in diabetic foot syndrome (DFS) is an important surgical problem. In addition to pain syndrome, infectious complications, risk of amputation and mobility disorders, diabetic foot ulcers are also associated with serious economic, social and psychological consequences. In the treatment of skin and soft tissue defects in patients with diabetes mellitus (DM), various methods of drug administration are proposed. However, there is still no single algorithm for choosing dressings and no single treatment algorithm to achieve maximum wound healing rate and reduce the incidence of patient disability. AIM: To evaluate the effect of dressings with granulated sorbent and modern antiseptic means on the treatment of wound defects in patients with DFS. MATERIALS AND METHODS: To determine the sample size an a priori analysis of power was conducted. Thirty three patients aged above 18 with type 2 DM and DFS were examined and treated in the ambulatory setting. The patients were randomly divided into 2 groups: group 1 was treated with the proposed method using chlorhexidine-dioxidine mixture (CDM) and granulated sorbent, group 2 received treatment with standard methods using antiseptic dressings with iodophor solutions (povidone-iodine, betadine, iodopyrone) and levomekol/levometil ointments. RESULTS: The duration of treatment averaged 17.27±3.96 weeks. The duration of the confirmed type 2 DM averaged 11.97±6.10 weeks. The dynamics of each wound healing was calculated based on the initial wound size and also the healing rate of each particular wound. In the main group the ratio of the healing rate to the initial wound area was 0.069±0.026. In the control group the parameter was lower: 0.055±0.006. A program was developed that permits calculating the predicted wound healing rate in less than 1 minute depending on the initial dimensions and type of dressing used. CONCLUSION: Elucidation of the importance of the influence of the used type of dressing on the duration of treatment and course of the wound process helps correct the management tactics for patients with DM to achieve better wound treatment results. The impact of dressing type can be assessed using a program developed for the study to estimate the predicted wound healing rate based on the initial wound size. This program enables the selection of the optimal dressing for each individual patient and wound.
