Background: Preterm premature rupture of membranes (PPROM) is a condition of rupture of the amniotic membranes without signs of delivery that occurs before 37 weeks of gestation. Group B Streptococcus (GBS) infection is one of the risk factors for preterm PPROM, but bacterial culture examination requires facilities and time to analyze culture results, so a simpler examination method is needed. This study aimed to evaluate total leukocytes and neutrophil-lymphocyte ratio (NLR) to predict GBS infection in pregnant women with PPROM. Methods: This study used a case-control analytical design involving 64 samples of PPROM pregnant women treated at Prof. I.G.N.G Ngoerah General Hospital, which were divided into 2 groups, 32 samples with GBS infection as the case group and 32 samples without GBS infection as the control group. Normality tests, univariate and bivariate analysis, and ROC curves were carried out to assess the ability of parameters to predict GBS infection. Results: The mean maternal age was 27.33, the dominant gestational age was 35 weeks (20.3%), primigravida (51.6%), with the highest number of parities was 0 (nullipara) (62.5%), and without a history of PPROM (68.8%). The mean of total leukocyte levels and NLR showed significant differences. Leukocyte levels ≥ 15,000 increased the risk of GBS infection 33 times (OR=33.00; 95%CI=6.57-165.89 p-value<0.001), while NLR≥4.49 increased the risk of GBS infection 3.86 times (OR=3.86; 95%CI=1.33-11.16 p-value=0.013) with sensitivity of 75% and specificity of 48.3%. Conclusion: High total leukocytes and NLR are risk factors for GBS infection in pregnant women with PPROM. Kadar leukosit ≥15.000 meningkatkan risiko infeksi GBS 33 kali lipat (OR=33,00 IK95%=6,57-165,89 p-value<0.001), sedangkan kadar RNL ≥4,49 meningkatkan risiko infeksi GBS 3,86 kali lipat (OR=3,86 IK95%=1,33-11,16 p-value=0.013) dengan sensitivitas RNL 75% dan spesifisitas 48,3%.
