Introduction: Basal cell carcinoma (BCC) is the most common type of skin cancer, with a predilection for areas exposed to ultraviolet radiation. The gold standard therapy has been established as surgical excision followed by histopathological confirmation. In nasal lesions, reconstruction is often required to maintain both function and cosmetic outcomes, one of which can be achieved using a bilobed flap. This report aimed to discuss the outcomes of patients with nodular basal cell carcinoma treated with surgical excision with a bilobed flap. Case: A 61-year-old male was reported with a blackish nodule on the right nasal region for four years, occasionally accompanied by bleeding. A history of chronic sun exposure without protection was identified. Dermoscopic examination revealed arborizing vessels and blue-gray globules. The lesion was excised with clinical margins, and the defect was closed using a bilobed flap. Histopathological examination confirmed nodular BCC, solid variant, with the closest tumor-to-resection margin ranging from 0.3–2 mm. On postoperative day 17, the wound had healed well without signs of infection or complications. Although the cosmetic result was considered satisfactory, narrow excision margins (<2 mm) were identified, which carry a higher recurrence risk compared to literature standards (≥3–5 mm). This finding highlights the importance of reconsidering the need for re-excision or long-term surveillance. Conclusion: The bilobed flap can be effectively utilized for nasal defect closure following BCC excision, providing favorable cosmetic outcomes. However, narrow excision margins should be taken into consideration due to the potential risk of recurrence, thereby necessitating long-term follow-up.
