Clinical efficacy analysis of orthoview software-assisted high tibial osteotomy combined with medial meniscus posterior root repair for degenerative medial meniscus posterior root tears
To compare the clinical efficacy of Orthoview software-assisted HTO combined with medial meniscus posterior root repair versus simple meniscus posterior root repair for the treatment of degenerative MMPRT. A retrospective analysis was conducted on 62 patients with degenerative MMPRT treated from January 2022 to August 2023. Patients were divided into an observation group (30 cases, Orthoview software-assisted HTO combined with meniscus posterior root repair) and a control group (32 cases, simple meniscus posterior root repair) based on treatment method. Propensity score matching was used to reduce selection bias. Assessment indicators included lower limb alignment parameters (HKA, MPTA), meniscus healing status (Stärke healing criteria), Lysholm knee function score, HSS knee score, ROM, VAS pain score, and complication rates. Follow-up was conducted for 12 months. There were no statistical differences in baseline characteristics between the two groups (P>0.05). At 12 months postoperatively, the observation group showed superior improvement in HKA and MPTA compared to the control group (P<0.001). The observation group had a significantly higher complete meniscus healing rate than the control group (P=0.002). Although the control group had better functional scores at 3 months postoperatively, the observation group showed significantly superior Lysholm scores, HSS scores, and ROM at 6 and 12 months postoperatively (P<0.001). VAS pain scores in the observation group were lower than those in the control group at all time points (P<0.001). Stratified analysis by varus deformity degree showed that patients with moderate to severe varus deformity (>10°) demonstrated more pronounced advantages with combined surgery. The observation group had a higher rate of clinically significant improvement than the control group (P=0.006). There was no significant difference in complication rates between the two groups (P=0.614). Orthoview software-assisted HTO combined with medial meniscus posterior root repair demonstrates significant efficacy in treating degenerative MMPRT. By improving lower limb alignment and reconstructing meniscus anatomy, this approach is superior to simple meniscus repair in pain relief, functional improvement, and meniscus healing. For patients with moderate to severe varus deformity combined with MMPRT who have failed conservative treatment, combined surgery should be the preferred treatment method.
