ANALYSIS OF INDICATORS CHARACTERIZING ORGANIZATIONAL ASPECTS OF CANCER CARE DELIVERY IN THE ARKHANGELSK REGION

Significance. Malignant neoplasms remain one of the most important problems in the field of medicine and public health and one of the most significant items of expenditure of the healthcare system. Despite the positive dynamics in the implementation of the regional project “Fight against cancer”, the region failed to fully achieve the targets. The subject of the study: early detection of malignant neoplasms, active detection of malignant neoplasms, the proportion of morphologically confirmed malignant neoplasms, one-year cancer survival rate, 5-year cancer survival rate. Purpose. To analyze the organizational aspects of cancer care delivery in the Arkhangelsk region. Material and methods. The study used statistical data for 2018-2023 according to federal statistical observation form No. 7 “Information on malignant neoplasms” for the Arkhangelsk region. All indicators were calculated with due regard to the statistical data of the Territorial Body of the Federal State Statistics Service for the Arkhangelsk Region on the average population. For comparison, data on the Northwestern Federal District and the Russian Federation were taken from the official reference materials of the P.A. Herzen Moscow Oncology Research Institute published in the public domain (“Malignant neoplasms in Russia in 2018-2023 (incidence and mortality)”, “The state of cancer care delivery to the Russian population in 2018-2023”), information of the Federal State Statistics Service, and the Passport of the Federal Project “Fight against cancer”. Results. Despite positive dynamics in the indicators of early detection of cancer and actively detected cases, which are the effectiveness indicators of cancer care organization, the values in the Arkhangelsk Region are lower compared to the similar indicators of the Northwestern Federal District and the Russian Federation as a whole, and the one-year survival rate is higher. The only indicator that not only shows positive dynamics, but also exceeds the national average is the share of morphologically confirmed diagnoses. Conclusion. The discrepancy between the level of indicators stated in the “Fight against cancer” program pinpoints problems in the organization of primary health care, including primary specialized cancer care in the Arkhangelsk region. A detailed analysis of the causes of organizational defects in cancer care delivery by medical organization and a systemic region-wide approach to their elimination will contribute to improving the quality of and access to cancer care, and, as a result, achieving the target indicators. Scope of application. Management decision making to improve the quality and accessibility of cancer care at the regional and institutional levels. Keywords: organization of cancer care delivery; early detection of malignant neoplasms; active detection of malignant neoplasms; the proportion of morphologically confirmed malignant neoplasms; one-year cancer survival rate; five-year cancer survival rate.
