P01.35.A SERUM GLIAL FIBRILARY ACIDIC PROTEIN (GFAP) AND EPIDERMAL GROWTH FACTOR (EGFR) AS PREDICTOR DIAGNOSTIC AND PROGNOSTIC ISOCITRATE DEHYDROGENASE (IDH) TYPE OF GLIOMA

Abstract BACKGROUND glioma is a brain tumor that originates from glial cells. The degree of malignancy of glioma has different prognostic values. Currently, there are no universal specific markers for glioma. Markers such as Glial Fibrillary Acidic Protein (GFAP) and Epidermal Growth Factor Receptor (EGFR) can provide insight into the diagnosis and prognosis of gliomas with Isocitrate Dehidrogenase (IDH) mutation. OBJECTIVE This study aims to evaluate the serum levels of GFAP and EGFR as predictors for IDH type diagnosis and prognosis in glioma. METHODOLOGY Prospective study was conducted with pre-operative serum samples from 70 patients. As imnunohistochemical analysis for IDH-1 R132H was performed to confirm on glioma tissues. Statistical analysis were conducted to determine the correlation between serum GFAP and EGFR levels with IDH type and glioma prognosis. RESULT There was significancy in correlation between serum GFAP levels with IDH positive patiens (p<0.032), whereas there was not significant correlation IDH positive with serum EGFR. There was trend of increasing serum EGFR levels corresponding with rising serum GFAP levels (p=0.000). In glioma grade IV, a serum GFAP value <= 2.15 ng/ml is 2.745 times more likely to be positive for IDH. Patients with low GFAP, EGFR serum and positive IDH had longer survival. There is a positive relationship trend in low GFAP serum, low EGFR serum and IDH with an increase in survival rate, although it is not statistically significant. CONCLUSION Serum GFAP levels are significantly associated with IDH positive in glioma patients, while serum EGFR level not directly linked to IDH status. Both are significantly correlated to survival outcomes in glioma grade IV. These findings suggest that GFAP and EGFR are useful as valuable biomarkers for glioma diagnosis and prognosis, aiding in personalizes patient management strategy.
