P17.09.A SYSTEMIC TREATMENT PATTERNS IN RECURRENT HIGH-GRADE GLIOMAS IN EUROPE AND ASIA: A REAL WORLD ANALYSIS

Abstract BACKGROUND High-grade gliomas (HGG) are aggressive tumors associated with high recurrence rates and poor survival outcomes despite multimodal treatment, including surgery, radiotherapy, and chemotherapy. Upon recurrence, therapeutic options become limited, and clinical decision-making is often influenced by regional practice patterns, healthcare infrastructure, and drug accessibility. Bevacizumab and investigational therapies show promise, but regional disparities persist. Understanding these patterns is essential to optimize care for recurrent HGG patients. MATERIAL & METHODS This real-world study utilized data from IQVIA’s Oncology Dynamics, a multinational, cross-sectional survey that captures anonymized patient-level information reported by oncologists. The objective was to assess systemic treatment patterns for recurrent high-grade gliomas (WHO grade 3 or 4) in Europe and Asia. Patients treated between January 2022 and December 2024 were included from selected countries in Europe (France, Germany, Italy, Spain, the United Kingdom, and Turkey) and Asia (China, Japan, South Korea, and India). RESULTS A total of 581 patients were included (467 from Europe and 114 from Asia). Bevacizumab monotherapy was more frequently prescribed in Asia (44%) compared to Europe (26%). Bevacizumab-based combination regimens were also more common in Asia (32% vs. 24%). Conversely, chemotherapy-based regimens were used more frequently in Europe (39%) than in Asia (21%). Other therapies accounted for 4% of cases in Asia and 11% in Europe. Country-level differences were notable. In Asia, bevacizumab monotherapy predominated in Japan (55%), while combination regimens were favored in China (59%). Chemotherapy use was lowest in China (15%) and moderate in Japan (28%). In Europe, chemotherapy was the dominant approach in the UK (89%) and Italy (44%), while bevacizumab monotherapy was most used in Spain (52%). Bevacizumab-based combinations were most prevalent in Turkey (63%) and France (55%). Italy showed the highest use of other regimens (48%). CONCLUSION This study reveals substantial regional variation in the systemic treatment of recurrent HGG across Europe and Asia. Bevacizumab monotherapy and combinations are more frequently used in Asian countries, particularly Japan and China, whereas chemotherapy regimens dominate in Europe, especially in the UK and Italy. These differences likely reflect disparities in national guidelines, drug access, and physician preferences. Recognizing these variations is essential to harmonizing treatment approaches and guiding future research aimed at optimizing outcomes in recurrent HGG.
