P070 Replacing sedatives with behavioural interventions in complex insomnia

Abstract Introduction This case study explores sleep outcomes in a patient with multiple comorbidities after withholding sedative medications and introducing behavioural strategies during PSG. Methods A 34-year-old male with chronic pain, neurodevelopmental disorder/traumatic brain injury, insomnia, and polypharmacy underwent two PSGs. Study 1 included evening medications; Study 2 replaced medications with meditation, yoga, breathwork (including Wim Hof breathing), and interpersonal stimulation. Results Discussion Withholding centrally acting sedatives and replacing them with behavioural interventions yielded modest improvements in sleep efficiency and REM quality in a patient with extensive sleep-interrupting comorbidities. This suggests a potential role for non-pharmacological strategies in even the most complex insomnia cases, though individualised approaches are critical.
