OVARIAN TORSION IN THE SECOND TRIMESTER OF PREGNANCY
Ovarian torsion during pregnancy is an uncommon but urgent cause of acute abdomen, particularly in the second trimester. We report the case of a 23-year-old multiparous woman, with a history of previous lower segment cesarean section, presenting at 14 weeks of amenorrhoea with severe right iliac fossa pain, adnexal tenderness, and a palpable cystic mass. Ultrasound with Doppler revealed a 10 × 5 cm right ovarian cyst without detectable vascular flow and a single live intrauterine pregnancy of 12 weeks' gestation. Emergency laparotomy under regional anesthesia identified a right ovarian cyst twisted three times around its pedicle. Detorsion and cystectomy were performed, and histopathology confirmed a benign serous cystadenoma. The postoperative period was uneventful, and the patient was discharged after five days. This case underscores the importance of prompt diagnosis and surgical management of adnexal masses in pregnancy to prevent maternal and fetal complications.
