Особенности клинического течения гиперпластических процессов эндометрия в разных возрастных группах
There are a number of differences in both the clinical course and the sonographic examination data of patients with endometrial hyperplasia in the reproductive period and perimenopause. The symptoms in both age groups were approximately the same and were expressed by pain and bleeding. If in the reproductive age endometrial hyperplasia was most often combined with endometriosis, then in the climacteric period, the most frequent concomitant gynecological pathology was uterine myoma. Consequently, the diagnosis of endometriosis in the reproductive age and uterine myoma in the perimenopausal period require mandatory additional examination in order to detect endometrial hyperplasia. The frequency of concomitant pathology in the reproductive age was low, while in the perimenopause period, concomitant diseases were quite often accompanied by endometrial hyperplastic processes. It is important to note that the classic triad of obesity, hypertension and diabetes mellitus, as is known, is quite often noted in the old age. However, in the reproductive age, this triad loses its significance, according to ultrasound data. In particular, in the presence of hypertrophy at a young age, polycystic syndrome is detected more often, while ovarian cysts are detected twice as often in patients with hypertrophy at an old age. There are no clear echographic signs of endometrial hyperplasia in reproductive age, and therefore the diagnostic significance of ultrasound for detecting endometrial hyperplasia in women of this age group is still limited. It should be noted that endometrial hypertrophy is not always recorded during sonographic examination, and in this case, hysteroscopy has acquired particular importance as a diagnostic method.
