Total Annulus Excision Tympanoplasty Outcome for Large Central Perforation – Our Institutional Experience
Aim and Objectives: The study aimed to compare the postoperative outcomes between conventional tympanoplasty and total annulus excision tympanoplasty in patients with large-sized central perforation having inactive mucosal chronic otitis media (COM). The objectives were to compare graft uptake, hearing improvement, and postoperative complications following total annulus excision tympanoplasty and conventional underlay tympanoplasty. Study Design: Case–control study. Materials and Methods: Patients with COM (inactive mucosal type) aged between 15 and 55 years having large central perforation attending our outpatient department were assessed, and eligible patients who gave consent for the study were included. Sixty patients were taken for the study, out of these 30 patients were chosen for total annulus excision tympanoplasty (Group A), and 30 patients were chosen for conventional underlay tympanoplasty (Group B). Results: Patients in Group A (total annulus excision tympanoplasty) showed better graft uptake, hearing improvement, and less postoperative complications when compared with Group B (conventional underlay tympanoplasty). Conclusion: Total annulus excision tympanoplasty may be preferred over the conventional techniques in patients with large central perforations. Furthermore, it can prevent the development of granular myringitis effectively.
