Langetermijneffecten bij patiënten behandeld met platinum-gebaseerde chemotherapie voor testiscarcinoom

Long-term adverse health outcomes among patients with testicular cancer treated with platinum-based chemotherapy Background: Testicular cancer is the most frequent malignancy amongst males between the age of 20 and 35. The prognosis greatly increased after the introduction of platinum-based chemotherapy, nowadays achieving 10-year overall survival rates of >95%. However, this also necessitates a thorough knowledge of long-term adverse events, to pay attention to specific needs during follow-up, which are often neglected. Furthermore, the evidence is scarce and currently underexposed. We’ve reviewed the potential long-term adverse effects related to platinum-based treatment regimens and aim to formulate recommendations for the foundations of a survivorship care plan. Methods: We performed an extensive literature search in Pubmed and Embase, including articles published up to November 2023. Studies recruiting patients treated before 1970 were excluded since they might contain obsolete treatments. Reviews were only considered if they contained additional information. Thirty-four articles were withheld. Results: Patients treated with platinum-based chemotherapy showed an increased risk for secondary malignancies, cardiovascular diseases, neuropathy and ototoxicity, hypogonadism and sexual dysfunction. The risk for lung and renal toxicities was rather limited except for certain subgroups. Testicular cancer survivors may also experience psychosocial distress. Conclusion: Testicular cancer survivors are exposed to several long-term adverse events. They need high-quality survivorship care. To date, the most important recommendations are addressing potential long-term effects during consultation, adequate monitoring of the cardiovascular risk profile, maintaining a healthy lifestyle and participating in screening programs that also apply to the general population. Further research is needed to formulate more targeted recommendations.
