Desenlaces Cardiovasculares Adversos a 5 Años en Pacientes Evaluados con Monitoreo Ambulatorio de la Presión Arterial y Ecocardiografía Transtorácica

Introduction: Hypertension is the most prevalent cardiovascular disease worldwide. It contributes to morbidity and mortality due to its association with target organ damage, establishing a close relationship between elevated blood pressure and risk of adverse cardiovascular events. There is limited literature available about echocardiographic findings related to ambulatory blood pressure monitoring with long-term cardiovascular outcomes. Objective: to estimate 5-year frequency of adverse cardiovascular outcomes in a group of patients evaluated with ABPM and transthoracic echocardiography in Colombia. Methods: Descriptive, observational study conducted in patients with hypertension evaluated between June 2018 and December 2019 using ABPM and transthoracic echocardiography. Patients were assessed via telephone survey, and their medical records were reviewed to evaluate cardiovascular outcomes five years after their initial evaluation. Descriptive statistical analyses were executed with Jamovi 2.6. Results: a total of 60 patients were included. The most frequent 5-year adverse cardiovascular outcomes were heart failure (32%) and acute myocardial infarction (32%). A possible association was found between the reverse dipper phenotype and the occurrence of acute myocardial infarction (p = 0.032). Conclusion: this study demonstrated a possible association between the reverse dipper phenotype and the occurrence of acute myocardial infarction. This finding should prompt clinicians to intervene early in search of potentially reversible causes associated with this hypertension phenotype.
