Preventable medication-related readmissions involving older adults: a retrospective cohort analysis

Nearly one-quarter of 30-day readmissions involving older adults were medication-related and potentially preventable. The primary contributing factors were prescribing and non-adherence issues. The study highlighted common causes of such readmissions and underlines the need for further research to determine which interventions - such as medication reviews or improved care transitions - are most effective in addressing them. Identifying high-risk patients will also be essential to optimise the use of healthcare resources.
