First-in-Human Experience of Robotic-Assisted Transcatheter Aortic Valve Implantation with Femoral Artery Approach: A Case Report
Abstract Background Robotic-assisted transfemoral TAVI remains unreported. We present the first successful case using a novel robotic system. Case Summary A 70-year-old male with severe symptomatic aortic stenosis (mean gradient 52 mmHg, LVEF 48%) underwent robotic-assisted TAVI via femoral access. The procedure was remotely controlled from a radiation-shielded console using a Taurus Elite® 29mm valve. Key steps—wire navigation, balloon valvuloplasty, and valve deployment—were completed robotically in 11 minutes. Post-procedural TTE showed a peak gradient of 12 mmHg and LVEF improvement to 58% at 1 month, with no complications. Discussion This pioneering case confirms the feasibility of robotic TAVI, eliminating operator radiation exposure while maintaining procedural efficacy. The system’s integrated control may standardize complex valve deployments.
