Robotic-assisted abdominal cerclage: In the pregnancy interval
Cervical incompetence, marked by premature cervical shortening and dilation, is a leading cause of second-trimester pregnancy losses and preterm births. While vaginal cerclage is commonly employed, its failure may necessitate a more definitive approach. Robotic-assisted transabdominal cerclage (RA-AC) offers enhanced precision, reduced blood loss, and quicker recovery, making it a valuable alternative, especially in patients with prior failed cerclages. We present the case of a 33-year-old woman with a history of recurrent pregnancy losses due to cervical insufficiency, including two failed vaginal cerclages and one preterm delivery at 28 weeks. RA-AC was performed during the periconceptional period using the robotic system, with an uneventful postoperative course and next-day discharge. The patient subsequently conceived via intrauterine insemination and delivered a healthy full-term female infant weighing 2.7 kg at 37 weeks by lower-segment cesarean section. This case highlights the efficacy and safety of RA-AC during the periconceptional period in improving pregnancy outcomes in women with cervical incompetence with previous failed vaginal cerclages.
