TREATMENT TACTICS FOR PATIENTS WITH INJURIES TO THE ACROMIOCLAVIC JOINT
The choice of methods for treating acromioclavicular joint (ACJ) injuries is a pressing issue in modern traumatology. The shortcomings of existing methods for treating ACJ injuries indicate the need for in-depth research to find optimal methods for correcting injuries in this location. Purpose of the study. To present the results of surgical treatment of 90 patients with ACJ injuries aged 18 to 70 years. Materials and methods. The article presents the results of surgical treatment of 90 patients with ACJ injuries aged 18 to 70 years. The patients were divided into two clinical groups: 40 (45.5%) patients were assigned to the main group, 50 (55.5%) to the control group. Patients in the main group were treated using a new method for restoring the ACJ, a new device for limb fixation and early rehabilitation, for which there is a patent of the Republic of Kazakhstan and copyrights for inventions. In the treatment of patients in the control group, a traditional method was used with a hook-shaped plate without restoration of the acromioclavicular joint ligaments. Results. The remote result was studied in 73 (81.0%, n = 90), including 36 (90%, n = 40) patients in the main group and 37 (74%, n = 50) in the control group. The frequency of positive treatment results in the main group was 91.6% (72.2% excellent and 19.4% good). The incidence of excellent and good results in the main group was higher than in the control group by 1.7 and 1.2, respectively (p < 0.001). Satisfactory treatment results in the main group were 4.8 times lower than in the control group (p < 0.001). The proportion of patients with unsatisfactory results in the control group was 4.7 times higher than in the main group (p < 0.001). Conclusions. Clinical testing of a new combined method of acromioclavicular joint restoration, a new device for early rehabilitation and a new device for limb fixation increased the frequency of excellent and good treatment results by 1.7 and 1.2 times, respectively, reduced the frequency of satisfactory by 4.8, unsatisfactory results by 4.7 times compared with the traditional method of treatment.
